"In the UK, the use of Performance and Image Enhancing Drugs (PIEDs)—including anabolic steroids, peptides, and SARMs—has transitioned from elite athletics into the general population, with estimates suggesting between 1 and 3 milion active users, the vast majority of whom are non-athletes.
While possession for personal use is currently legal under the Misuse of Drugs Act 1971, the market is entirely unregulated, leading to a high prevalence of counterfeit, mislabelled, or contaminated products. In addition, rogue advisors, frequently operating through platforms like TikTok and Instagram.
There is no effective regulation of these channels, and these advisors frequently advise end-users to experiment with veterinary drugs, research compounds, and enormous doses of PIEDs. SafePulse was founded to research safe protocols and train coaches, clinicians, and researchers to help treat the harm done to individuals by dangerous coaches and compounds."

As a leading medical clinic in Chelsea, we’re proud to be at the forefront of harm reduction and modern, evidence-based interventions for performance and health. We’re delighted to act as a major clinical partner in supporting Safe Pulse.

“During my career as a Drug Expert Witness, I’ve seen first-hand the damage caused by illicit drug markets, misinformation, and the absence of credible oversight. Performance- and image-enhancing drugs sit in a grey area that is often misunderstood, poorly regulated, and rarely discussed openly, despite the very real risks involved. What stands out about SafePulse is their willingness to confront this issue head-on — shining a light on a space that many avoid, while offering structured, evidence-led solutions rather than fear or judgement. Initiatives like this are essential if we are serious about reducing harm, protecting individuals, and preventing long-term consequences that are otherwise entirely avoidable.”

“In major trauma and regenerative medicine, I see the consequences of poor decision-making every day. In the current landscape, performance-enhancing drugs and advanced interventions are more accessible than ever, yet the quality of guidance surrounding their use is deeply concerning. While these tools can be transformative when used appropriately, the harm caused by misuse, misinformation, and poorly designed protocols can be devastating. It is imperative that evidence, data, and genuine clinical expertise guide practice in this space. SafePulse represents a rare and necessary standard — prioritising safety, harm reduction, and long-term health through rigorous, actionable protocols. This is exactly the kind of work that prevents avoidable injury and saves lives.”

As a UK social worker and a trustee at a drug and alcohol agency, I routinely see how health risk escalates when people rely on informal advice, inconsistent monitoring, and misinformation in unregulated substance markets. SafePulse’s core strength is the way it bridges research and real-world application through evidence-based protocols focused on peptides, SARMs, and growth hormones. The platform is structured around clear pillars that translate into practical safeguards: harm reduction, safety monitoring, responsible use, and longevity. I particularly value the emphasis on safety monitoring, including biomarker tracking, because it supports earlier risk identification and better-informed adjustments over time. I also value the focus on monitoring because it reduces avoidable complications, including harms that can become effectively iatrogenic when people follow unreliable “protocols” presented as medical guidance. SafePulse’s education-first, transparent, research-driven stance also has clear mental health relevance. It reduces uncertainty, supports realistic expectations, and introduces structured checkpoints that can interrupt compulsive escalation and emotionally driven decision-making. Clear monitoring guidance and defined red flags also make it easier to recognise when professional input is needed, supporting psychological stability alongside physical safety.

“As the owner of Renegade and someone who’s trained champions, I see exactly how fast misinformation spreads in combat sports — especially around PEDs, peptides, and ‘protocols’ that get shared like they’re harmless. The problem isn’t that athletes want to improve; it’s that too many are making high-risk decisions with zero real oversight, monitoring, or understanding of long-term consequences. That’s why I respect what SafePulse is doing. They bring evidence, structure, and safety thresholds into a space that’s usually driven by ego and guesswork — helping fighters make smarter, health-first choices that protect performance today and longevity for the years after the lights go out.”

Owner of renegade - trainer of champs
I was diagnosed with chronic kidney disease in 2015. At the time, I was still heavily focused on performance and progression, and I ignored warning signs that, in hindsight, mattered far more than I was willing to admit. When I was using PEDs, my blood pressure was high, and I simply chose not to engage with it. Like many people in performance-driven environments, I told myself I would deal with the health side later. In 2018, I underwent a kidney transplant. That moment fundamentally changed my understanding of health. A transplant is not a cure—it is a lifelong condition that requires continuous monitoring, medication, and responsibility. Since then, everything about how I train, eat, hydrate, recover, and manage stress has had to be considered through the lens of long-term health. Looking back, I genuinely believe that if an organisation like Safe-Pulse.org had existed when I was first diagnosed—bringing together practical science, clinical oversight, and lived experience—I might have learned earlier how to recognise risk, take it seriously, and make different decisions. Education, monitoring, and timely guidance matter. One of the most significant problems I see now is the sheer volume of false information and “bro science” online. People are often left making serious health decisions based on anecdotes, influencer content, or commercial narratives rather than credible evidence. At the same time, much of the academic and policy world remains focused on condemning performance enhancement outright rather than developing the science, frameworks, and services needed to support health, safety, and long-term outcomes for people already engaging in these practices. Since my transplant, Safe-Pulse.org and its partners have played a central role in monitoring my health and supporting me in training and competing safely. Their team—bringing together doctors, endocrinologists, urologists, and performance scientists—is genuinely unique. I have not encountered another organisation or company that comes close to the depth of support, integration of expertise, or level of practical and scientific understanding they provide. Their work reflects a rare combination of clinical rigour, performance knowledge, and real-world awareness. Their approach is grounded in Performance, Uplift, Longevity, Safety, and Education (PULSE)—and that balance was precisely what was missing in my earlier journey. Performance still matters to me, but it now sits alongside longevity, safety, and informed decision-making. Competing at Mr Olympia in 2025 was the pinnacle of my career, but it was only possible because my health came first. That experience reinforced something I now firmly believe: ambition and severe health conditions can coexist, but only when performance is supported by proper education, monitoring, and respect for the body. I fully endorse and support the work of Safe-Pulse.org and their team. Their model shows that it is possible to move beyond judgment and misinformation and toward evidence-informed, ethical support that genuinely protects health—not just for competition, but for life.

I fully support the initiative led by Safe-Pulse.org and its partners, including TransformNow.co.uk. The growing use of performance-enhancing drugs is a matter of serious concern for society as a whole. These substances can act as gateway drugs and are frequently associated with wider social harms alongside significant physical and mental health risks. For many years, this issue has remained largely undiscussed, or where discussed, has been insufficiently addressed. In this context, the forward-thinking and proactive approach demonstrated by Safe-Pulse.org and its partners is both timely and commendable, and deserves recognition and support.

“Working in frontline pre-hospital and urgent care, I regularly see the consequences of misinformation, delayed intervention, and unmanaged health risk. Many of the emergencies we attend are preventable when people have access to accurate information, appropriate monitoring, and clear safety thresholds. What I value about SafePulse is that it applies the same principles we use in emergency medicine — evidence-based decision-making, risk stratification, and early identification of harm — to a space that is often poorly understood and rarely governed. Their structured, safety-first approach doesn’t just educate; it actively reduces risk, prevents escalation, and has the potential to save lives.”

As an international lawyer, I push my body to its limits daily—long hours, constant travel, jumping on and off planes, and keeping up with a young family. Outside of work, I demand just as much from myself in the gym and the outdoors. Yet despite having more access than ever to fitness and health advice, staying healthy has never felt harder. We live in a world flooded with disingenuous coaches, recycled training plans, trending supplements, and pop-up health “professionals” armed with blood tests but little accountability. The noise has overtaken the truth. That’s why Safe Pulse matters. Not just as another option—but as a vital one. One glance at the current Ozempic trend shows the state of modern health shortcuts and how disconnected many approaches have become from real wellbeing. The team at Safe Pulse cuts through the misinformation by providing real, reliable data on your health, backed by safe, evidence-based guidance and meaningful treatment plans. Their advice isn’t driven by trends—it’s driven by truth, care, and long-term results. Your health is your greatest asset. Safe Pulse helps protect it—so you can stay the strongest, healthiest, and most capable version of yourself, for both your career and the people who rely on you most.

“Grappling and combat sports have experienced the impact of AAS. I’ve been immersed in the study of this class of drugs since I was a young man. In our current era, information on androgens and other PEDs has never been more accessible; however, the sheer volume of misinformation is deeply worrying. Whilst these compounds can be life-changing at a therapeutic level for many, the consequences of abuse can be catastrophic. Some of the protocols I’ve seen recommended are absolutely terrifying. It’s imperative, in my humble opinion, that academic experts with a genuine duty of care are consulted. Imran and his colleagues are brave enough to shine a light on a topic that is rarely discussed, while also providing real, practical solutions — and they have, and will continue to, save lives.”

Given the rapid rise in the use of performance- and image-enhancing drugs, much of it driven by misinformation and a lack of clinical oversight, I am genuinely glad that Safe-Pulse.org exists. The long-term endocrine, cardiovascular, and mental health risks — particularly for vulnerable young men — are significant and represent a growing future burden on the NHS. Safe Pulse is the only organisation I am aware of taking a structured, evidence-led approach to education, harm reduction, and responsible intervention in this space, and their work is both necessary and timely.

“Having spent decades working across exercise science, sports nutrition, sports medicine, and bodybuilding, I’ve seen both the benefits and the very real risks associated with performance and physique enhancement. While evidence-based approaches can improve performance and health, misinformation, poor supplementation practices, and unsafe PED use remain widespread. What I respect about SafePulse is their commitment to evidence, transparency, and harm reduction — translating complex science into practical guidance that prioritises safety, longevity, and informed decision-making. This kind of work is critically needed in the fitness and physique space, and it has the potential to genuinely protect athletes and save lives.”

What I value about SafePulse is that it applies the same principles we use in emergency medicine — evidence-based decision-making, risk stratification, and early identification of harm — to a space that is often poorly understood and rarely governed.
